We had the good fortune of connecting with Sarah Hormachea and we’ve shared our conversation below.

Hi Sarah, can you walk us through the thought-process of starting your business?
At the core, my decision to start my own business came from a very real gap I kept seeing in healthcare, especially in diabetes care.

Across the country, and very much here in Colorado, there is a serious shortage of healthcare providers. Many people wait weeks or even months to get the care they need. For someone living with diabetes, the average wait time for a new endocrinology appointment is around 10 weeks, and in some areas it can stretch to six months or longer. During that time, people are often referred from primary care to specialty care without much hands-on support in between.

At the same time, diabetes care has become more complex. New medications, technologies, and clinical guidelines are advancing quickly, but clinic visits are getting shorter, care teams are stretched thin, and burnout is high. That combination can leave both patients and providers feeling stuck, even when everyone wants better outcomes.

I kept asking myself a simple question: what if more diabetes support could happen at the primary care and community level, rather than waiting months for specialty care?

That question became the foundation for my business. Through Sarah Hormachea: Diabetes Care and Education LLC, I partner with clinics, health systems, and community organizations to strengthen how diabetes care is delivered. My work focuses on filling gaps through consultative and contract services like diabetes education program development, insulin and pump therapy training, nutrition therapy, and project-based clinical support.

The goal is not to replace existing care teams, but to extend them. By building scalable, patient-centered solutions, I help organizations support people with diabetes sooner, more consistently, and in ways that reduce strain on already overwhelmed systems.

Let’s talk shop? Tell us more about your career, what can you share with our community?
My career in diabetes care and education has been shaped by a mix of clinical training, curiosity, and a willingness to step outside traditional paths when they no longer fit. Before launching my business, I worked in a large academic endocrinology practice, providing care and education to people living with diabetes and other complex metabolic conditions. I loved the work itself, but over time I became increasingly aware of systemic gaps. Care was growing more complex, visits were getting shorter, and both patients and clinicians were being asked to do more with fewer resources.

The turning point came shortly after I returned from maternity leave. Balancing long clinical days, a lengthy commute, and the emotional demands of specialty care made it clear that something needed to change. Leaving a traditional hospital role was not easy. It took months of planning, financial planning, and honest reflection about what I wanted my career and family life to look like. I spent nights and weekends lining up contract work before finally taking the leap into entrepreneurship.

What sets my work apart is its focus at the intersection of clinical care, systems thinking, and relationship-building. Through my consulting practice, I partner with clinics, health systems, and community organizations to strengthen diabetes care delivery, whether that means building or expanding education programs, supporting advanced insulin and pump training, integrating new therapies and technologies into clinical workflows, or mentoring clinicians navigating their own career transitions. I also continue to work directly with patients, which keeps my approach grounded in real-world needs.

I am most proud of building a career that prioritizes quality over hustle. Earlier in my career, I was driven by volume and productivity. Now, I am much more intentional about focusing on high-impact projects and meaningful connections. Those moments when a clinician gains confidence, a program finally clicks into place, or a patient has a true “light bulb” moment are what keep me energized.

Professionally, I am also deeply proud of my leadership and advocacy work, particularly within organizations like Association of Diabetes Care & Education Specialists. Founding and leading the Entrepreneurship Community of Interest came from recognizing a gap and helping create a space where none existed before. It reinforced an important lesson: if you do not see what you need, sometimes the most powerful thing you can do is help build it.

The biggest lessons I have learned are to stay flexible, revisit your offerings often, and not wait for everything to be perfect before getting started. Most successful businesses and services evolve through iteration. You do not need to be everything to everyone. You just need to stay true to your north star and the guiding mission of your work.

If you had a friend visiting you, what are some of the local spots you’d want to take them around to?
Now that we have a little one at home, our family are big fans (and members) of the Denver Zoo, the Denver Museum of Nature & Science, and Denver Botanic Gardens. We are usually at one of these spots at least once a week throughout the year. I also have a special place in my heart for the Congress Park neighborhood and consider us regulars at Shells and Sauce, Blue Pan Pizza, and Under the Umbrella. And of course, no trip to Denver is complete without a visit to Red Rocks and a Rockies game.

A huge thank you to Jenny Madrid and the UCHealth Anschutz diabetes educator team. They are an incredible group of dietitians and nurse diabetes educators dedicated to supporting and empowering people living with diabetes.

I also want to thank the local Colorado chapter of the Association of Diabetes Care & Education Specialists (ADCES) and its volunteer members for fostering my passion for leadership and advocacy.
